Reliable graph discovery during refresh

MCP graph reads can continue to use ready snapshots while refresh maintenance runs in the background. Responses now retain bounded recovery state when a refresh cannot complete, so agents can distinguish available discovery from work that still requires current evidence.

Safer background recovery

Watcher recovery now coalesces repeated scoped resume requests, keeps only the latest pending target, and safely hands work off even when the caller is interrupted. This avoids duplicate resume work while retaining explicit failure metadata for recovery.

Clearer agent contracts

Packaged skills now show complete code-graph calls and an ordered memory closeout flow, including the distinction between the required handoff and optional Knowledge Delta review. The remember_context MCP schema also makes clear that handoff and smoke memories do not support authored or regenerated keywords.
