More focused code-anchor refreshes

Refreshing deferred code anchors now avoids starting background embedding work or historical prewarming solely for an anchor refresh. Normal indexing continues to use its existing background vector behavior.

Maintenance

Threadnote now uses Effect 4.1.2 internally.
